For Patients

On the day of your appointment please bring:

• Referral from your doctor
• Copies of recent tests results and hospital discharge summaries
• Relevant x-rays, CT/MRI/PET scans, angiograms
• List of current medications
• Medicare/DVA/Pension/Private Health Fund cards

Unfortunately due to the nature of cardiothoracic surgery there are often emergencies resulting in appointment postponement. If this occurs you will be contacted as soon as possible and rescheduled within a week.
